Bring your photo identification with you on the day of donation. FDA requires a photo I.D. in order to process your paperwork. The I.D. must have a birthday or social security number on it. If you have a donor card you may use that for registration. If you age 16 or 17, you must have a signed parental consent form. Parental Consent Form
Eat well and drink plenty of water prior to donation – this will increase the chances of a successful donation.
Fill out the DHF, Donor History Form, at time of registration. This form helps our team to determine if you are eligible to donate blood. Many factors can impact your eligibility such as travel, health conditions, lifestyle and medications. The Donor History Form is a controlled document that is mandated by the FDA for all blood banks.
Relax and make your donation. The donation process takes about one hour to complete. After completion have a seat in our waiting area and enjoy some snacks to ensure that there are no post donation side effects.
Schedule your next appointment. You can donate whole blood every 56 days, platelets every 7 days and plasma every 4 weeks.
